The development of newer surgical procedures, increasingly complex patient care in the operating theater and technical progress have led to the need for highly qualified personnel in the surgical service. As a result, in the 1990s, training for surgical assistants (OTAs) was introduced in Germany.

OTAs mainly work in surgical services in hospitals. There are other jobs in outpatient surgery centers as well as in emergency outpatient clinics and endoscopy departments in hospitals. OTAs know the surgical procedure like the surgeons themselves. They provide instrumentation during an operation, prepare all medical equipment, set up the instruments and hand them over. OTAs also have the task of providing additional devices, instruments or other required medical products, receiving examination material and relieving colleagues during instrumentation breaks.
